How do I submit a ticket or request

Overview

This outlines the process for submitting a ticket or request for services.

Steps

1. On the Homepage, select one of the boxes that aligns with the needed services.
   • For example, “Community Health & Wellness Services” if needing assistance to receive Mental Health Services or Vaccinations.
2. Fill out the form with the necessary information and click “Submit”
3. That’s it!
   • Each submission generates a ticket that is assigned to one of our L.I.F.E. Agents for review.
   • Upon review, the L.I.F.E. Agent will direct the ticket to the proper organization and
